Variations in the Measurement of Weaning Parameters of Mechanical Ventilation in São Paulo Hospitals

SUMMARY
BACKgRoUnD AnD oBJECTIVES: The objective this study is characterize the variability of methods used for obtaining weaning parameters in São Paulo hospitals.
METHoDS: A research questionnaire was conducted among physiotherapists from 9 hospitals. Fifty-three anonymous physiotherapists answered 32 questions of multiple choices distributed in 4 pages. The answers was tabulated and analyzed through absolute values and percentage. In the recognition of weaning parameters, the respiratory frequency was the only parameter cited in all the answers.
RESUlTS: Almost all (94%) obtained measurement with the patients breathing under previously adjusted fraction of inspired oxygen with patient connected to the ventilator in pressure support mode (91%). There was certain variability on the level of added pressure support (6 to 12 cmH20). There was wide variation in the time before recording the weaning parameter
(15 minutes). Measurements of parameters was done either with portable instruments or read from the ventilator display. The maximal inspiratory pressure had wide variation in the duration of airway occlusion (< 1 to 20 seconds), with the most frequent time frame being 2 to 4 seconds. Differences were noted between physiotherapists from the same hospital, as well as between different hospitals.
ConClUSIonS: There is great variation among physiotherapists when obtain weaning parameters. The results of this survey reinforce the need for standardization of all techniques, as well as continuous research about parameters that may better identify a patient’s ability
